Kathleen (Kathy to many) Ann Chase, 69, formerly of Chicago and Brookfield, died quietly and comfortably in the early hours of the morning on Sunday, Dec. 7, 2025. She worked as a secretary for a manufacturing company.
Kathy is survived by her son, Phil Chase; her stepdaughter, Charity (Tom) Gjersoe; her grandchildren, PJ, Payton, Brandon, and Carter (auntie grandma to Eli, Addie, and Ezra); her sister, Kris (Steve) Kahn; her brothers, Tom (Karen) Rafac and John Rafac; her goddaughters Becca (Joe) Lillig and Sarah (Brett) Eriksen; and many cousins, nieces, nephews, and family.
She was preceded in death by her husband, Walter; her daughter, Paulette (Mark) Healy; her mother, Florence Rafac (Locke); her father, Joseph Rafac; most recently her uncle, Tom Banasiak and aunt, Ruth Banasiak (Locke), as well as a number of cousins, aunts, uncles, grandparents, friends, family, and her great-nephew, Nolan Ross.
Visitation will be held on Wednesday, Dec. 17, from 8:30 to 9:30 a.m. at Hitzeman Funeral Home Ltd., 9445 31st St., Brookfield. Funeral Mass will be celebrated at St. Cletus Catholic Church, 600 W. 55th St., La Grange at 10 a.m. Interment at Queen of Heaven Cemetery.
